IRB/ERC Application Procedure for Faculty Member's Research

Any research involving human participants requires approval from the Institutional Review Board/Ethics Review Committee (IRB/ERC) before data collection. Additionally, Scientific Review Committee (SRC) approval is a prerequisite for the IRB/ERC ethics review, regardless of whether the project is funded or non-funded.

If your project has not yet been reviewed/approved by the SRC, please complete the SRC template (click here to download the SRC template), and send it along with your research proposal to mostafizur.rahman09@northsouth.edu. For externally funded projects, if the funding authority conducts a merit review and issues an approval letter confirming it, the SRC review may be waived upon submission of that approval letter.

Once the SRC application is approved, you will be informed through email. Then you may apply for the IRB/ERC ethics review. CTRG projects are already reviewed by the SRC. Therefore, you may directly apply for IRB review for CTRG projects.

The following documents are required to process the IRB application:

  3. Research Proposal
  4. Informed Consent form in English
  5. Informed Consent form in Bangla
  6. Questionnaire in English
  7. Questionnaire in Bangla
  8. Informed Consent Certificates of the Principal Investigator and all Co-Investigators; obtainable from:

For an expedited review, a memo addressed to the Chair, IRB/ERC, stating the reason for the expedited request, should be submitted along with the IRB application.

Documents numbered 1 to 8 should be submitted electronically to mostafizur.rahman09@northsouth.edu. Please ensure that the names and signatures of the Principal Investigator and all Co-Investigators are included in the IRB/ERC application template. Once your application is approved, an approval letter will be sent to your department.

IRB Amendment Procedure

For amendment, the following documents are required:

  1. IRB application template (select “Amendment” in “Type of Application”)
  2. A summary page listing all amendment items
  3. Amended documents with tracked changes or highlighted revised sections

IRB/ERC Application Procedure for Thesis Proposal

You may apply for IRB/ERC ethics review only after the proposal defense is completed.

The following documents are required for IRB/ERC ethics review:

  3. Thesis Proposal
  4. Informed Consent form in English
  5. Informed Consent form in Bangla
  6. Questionnaire in English
  7. Questionnaire in Bangla
  8. Your and your Supervisor's Informed Consent Certificates; obtainable from:

Please note that

  • The names and signatures of the Student Investigator and Supervisor are included in the IRB/ERC application template.
  • Include your supervisor in the email CC when submitting the application.
  • Mention the proposal defense date in the IRB application template.
  • Only application templates in Word or PDF format will be accepted; scanned, handwritten, or photo submissions will not be accepted.
  • Applications submitted after data collection or after thesis defense will not be accepted.

Documents numbered 1 to 8 should be submitted electronically to mostafizur.rahman09@northsouth.edu. Once your application is approved, an approval letter will be sent to you via email. You may also collect a hard copy of approval from the Office of Research-NSU (Room: ADM 625).

IRB/ERC Application Procedure for Students Research (other than Thesis)
